Medical Records Custodian
The custodian of medical records will be Dr. Wanda Spuhler. She may be contacted at office@spuhlermedicalassociates.com for all record requests.
Requesting Medical Records
Please email office@spuhlermedicalassociates.com with the following information:
- Full name
- Date of birth
- Email address
Record Delivery Options
- Email: Password-protected PDF sent to the email address we have on file for you. Please send a separate email with the password you would like us to use for your secure PDF.
- Memorial Hermann Medical Group: Records transferred electronically at your appointment after you sign a HIPAA-compliant request.
- Other providers: Have your new provider send us a HIPAA-compliant request; we will forward records directly to the provider.
